Functional gastroduodenal disorders / Functional dyspepsia
These two entities proposed by the Rome III committee to define FD should be used operatively(1):
Rome III diagnostic Criteria* for Postprandial Distress Syndrome(1):
| Must include one or both of the following symptoms: | |
| 1. | Bothersome postprandial fullness, occurring after ordinary sized meals, at least several times per week. |
| 2. | Early satiation that prevents finishing a regular meal, at least several times per week. |
* Criteria fulfilled for the last 3 months with symptom onset at least 6 months before diagnosis
Rome III diagnostic Criteria* for Epigastric Pain Syndrome(1):
| Must include all of the following: | |
| 1. | Pain or burning located in the epigastrium, at least of moderate severity, at least once per week. |
| 2. | The pain is intermittent. |
| 3. | Not generalized or expanded to other abdominal or chest regions. |
| 4. | Not relieved by defecation or passage of flatus. |
| 5. | Not fulfilling criteria for gallbladder and sphincter of Oddi disorders. |
*Criteria fulfilled for the last 3 months with symptom onset at least 6 months before diagnosis
Functional dyspepsia (FD) is defined as the presence of symptoms thought to originate in the gastroduodenal region, in the absence of any organic, systemic, or metabolic disease that can explain them(1).
Previous Rome committees have defined dyspepsia as pain or discomfort in the upper abdomen and have excluded reflux symptoms on the definition. Discomfort comprised a large number of nonpainful symptoms, including upper abdominal fullness, early satiety, bloating, or nausea(1).
New diagnostic criteria.
| Must include one or more of these symptoms: | |
| 1. | Bothersome postprandial fullness. |
| 2. | Early satiation. |
| 3. | Epigastric pain. |
| 4. | Epigastric burning. |
... And no evidence of structural disease (even at endoscopy) that is likely to explain the symptoms.
Although functional dyspepsia is defined as the presence of particular symptoms that originates in the gastroduodenal region, in absence of organic pathology, the Rome III committee has proposed, particularly for experimental purposes, that the term Functional dyspepsia (FD) should preferably be replaced by the newly defined diagnostic categories: postprandial distress syndrome (PDS), and epigastric pain syndrome (EPS)(1).
